ABSTRACT

BACKGROUND: Binge drinking is a pattern of alcohol abuse. Its prevalence and associated risk factors are not well documented. Heavy drinking, on the other hand, has a well-documented association with bereavement. This report uses a cross-sectional, population-based survey to estimate prevalence of bingeing and its association with new bereavement. Bingeing is defined as 4 or more drinks (women) or 5 or more drinks (men) in a 2-4-h setting. For the first time in 2019, the Georgia Behavioral Risk Factor Surveillance Survey (BRFSS) included a bereavement item: 'Have you experienced the death of a family member or close friend in the years 2018 or 2019?' METHODS: Georgia BRFSS is a complex sampling survey administered annually. It is designed to represent the 8.1 million people aged 18 years and older in the U.S. state of Georgia. Alcohol consumption patterns are routinely measured in the common core. In 2019, the state added a new item probing for bereavement in the prior 24 months predating the COVID-19 pandemic. Imputation and weighting techniques were applied to yield the population prevalence rates of new bereavement, bingeing, and their co-occurrence with other high-risk health behaviors and outcomes. Multivariate models, adjusted for age, gender, and race, were used to estimate the risk for other unhealthy behaviors posed by the co-occurrence of bereavement and bingeing. RESULTS: In Georgia, bereavement (45.8%), and alcohol consumption (48.8%) are common. Bereavement and alcohol use co-occurred among 1,796,817 people (45% of all drinkers) with a subset of 608,282 persons reporting bereavement combined with bingeing. The most common types of bereavement were death of a friend/neighbor (30.7%) or three plus deaths (31.8%). CONCLUSIONS: While bingeing is a known risk to public health, its co-occurrence with recent bereavement is a new observation. Public health surveillance systems need to monitor this co-occurrence to protect both individual and societal health. In a time of global bereavement, documenting its influence on binge drinking can support the work towards Sustainable Development Goal #3-Good health and Well-Being.

ABSTRACT

A significant proportion of South African municipalities, who hold the mandate for providing solid waste management (SWM) services for millions of South Africans, appear to be on the brink of collapse. On the frontlines of municipal failure, the city of Makhanda, following two decades of poor governance and mismanagement, has found itself unable to fulfil its mandate, with the state retreating on SWM service provision, and disruptions to waste management services becoming a daily reality. Drawing on embedded, qualitative fieldwork, this article examines how differently placed residents have experienced disruptions to SWM services. This work explores how residents of Makhanda's two halves: the affluent and predominantly white neighbourhoods in the west, and the poor, non-white townships in the east, have (or have not) adapted to manage and dispose of their own waste during periods of disruption. Findings suggest that disruptions to waste management service provision have been broadly experienced by residents. However, the consequences of interruptions to municipal collection have not been evenly borne, as more resourced, western residents have been more successful at managing their own waste disposal, while the residents of Makhanda's townships are less capable of coping, with affected communities coming to resemble a dumping ground, and residents having to adopt unsafe or environmentally harmful disposal practices. These findings are important because they shed light on the challenges of creating cleaner, more equal communities without healthy municipal participation in waste management services, while raising important considerations for a South Africa facing the possibility of widespread municipal collapse.

ABSTRACT

Multifunctional nature-based solutions (NBS) can help urban areas become more climate proof and adaptable and provide a range of societal goals. Alongside chronic impacts from climate change, the Covid-19 pandemic has illustrated the disruption that unexpected and acute shocks can bring to society. Measures such as NBS can help reduce the vulnerability of urban areas and increase resilience. Traditional infrastructure planning relies on strong business cases to demonstrate the economic value of a scheme. Numerous approaches assign economic value to the benefits from using NBS. However, this value is more than what can be accounted for by traditional finance methods, as there are many different perspectives on 'value'. Decision making processes for selection of NBS measures require stronger integration of these value perspectives. This paper considers these perspectives in the business models that are being used in the decision processes regarding use and selection of NBS. Examples are drawn from case studies in the EU Begin project and also from the Living with Water partnership in the UK, which illustrate how value perspectives can be included in business cases for NBS, also signposting the need to account for potential future changes using scenario planning.

ABSTRACT

Despite government efforts, many rural Pakistani women forgo regular antenatal visits, are unprepared for birth, and deliver at home or in private facilities, because they are dissatisfied with public health services. This study examined pregnant women's perceptions of public health hospital prenatal care to suggest areas for improvement. Using simple random sampling, 200 pregnant women visiting a secondary care public health facility in Sargodha District, Pakistan, were enrolled in a cross-sectional study. The quality of prenatal care was assessed using a structured and validated questionnaire. Descriptive analysis and multivariate linear regression stepwise models were used. Of participants, 52% consider the services to be of poor quality. Education, income, number of living children, and long waiting time influenced the perceived prenatal care quality in the study population. Stakeholders rated existing services as suboptimal, especially in terms of staff availability and time spent, which reduces service use. Facility managers and policymakers should work to improve the quality of services to satisfy patients, encourage them to use antenatal care, and improve the health of both mother and child, especially in rural areas.

ABSTRACT

Since the beginning of the Coronavirus disease (COVID)-19 pandemic in December 2019, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been responsible for more than 600 million infections and 6.5 million deaths worldwide. Given the persistence of SARS-CoV-2 and its ability to develop new variants, the implementation of an effective and long-term herd immunity appears to be crucial to overcome the pandemic. While a vast field of research has focused on the role of humoral immunity against SARS-CoV-2, a growing body of evidence suggest that antibodies alone only confer a partial protection against infection of reinfection which could be of high importance regarding the strategic development goals (SDG) of the United Nations (UN) and in particular UN SDG3 that aims towards the realization of good health and well being on a global scale in the context of the COVID-19 pandemic.In this review, we highlight the role of humoral immunity in the host defense against SARS-CoV-2, with a focus on highly neutralizing antibodies. We summarize the results of the main clinical trials leading to an overall disappointing efficacy of convalescent plasma therapy, variable results of monoclonal neutralizing antibodies in patients with COVID-19 but outstanding results for the mRNA based vaccines against SARS-CoV-2. Finally, we advocate that beyond antibody responses, the development of a robust cellular immunity against SARS-CoV-2 after infection or vaccination is of utmost importance for promoting immune memory and limiting disease severity, especially in case of (re)-infection by variant viruses.

ABSTRACT

An additional helicopter base has been built for the Government Flying Service at the former Kai Tak airport runway site in Hong Kong, China. The base allows high-rise development to take place at Tung Chung new town near the service's existing base at Hong Kong International airport, which would have restricted emergency helicopter operations during poor weather. The project involved construction of a take-off and landing pad, parking bays and hangar for two helicopters, plus an office building for the flight planning centre and air command and control centre. This paper describes how use of an NEC contract helped ensure close collaboration between the employer and contractor, leading to timely completion with zero accidents despite restrictions caused by the Covid-19 pandemic.

ABSTRACT

COVID-19 and infectious diseases have been included in strategic development goals (SDG) of United Nations (UN). Severe form of COVID-19 has been described as an endothelial disease. In order to better evaluate Covid-19 endotheliopathy, we characterized several subsets of circulating endothelial extracellular vesicles (EVs) at hospital admission among a cohort of 60 patients whose severity of COVID-19 was classified at the time of inclusion. Degree of COVID-19 severity was determined upon inclusion and categorized as moderate to severe in 40 patients and critical in 20 patients. We measured citrated plasma EVs expressing endothelial membrane markers. Endothelial EVs were defined as harboring VE-cadherin (CD144+), PECAM-1 (CD31 + CD41-) or E-selectin (CD62E+). An increase in CD62E + EV levels on admission to the hospital was significantly associated with critical disease. Moreover, Kaplan-Meier survival curves for CD62E + EV level showed that level ≥ 88,053 EVs/µL at admission was a significant predictor of in hospital mortality (p = 0.004). Moreover, CD62E + EV level ≥ 88,053 EV/µL was significantly associated with higher in-hospital mortality (OR 6.98, 95% CI 2.1-26.4, p = 0.002) in a univariate logistic regression model, while after adjustment to BMI CD62E + EV level ≥ 88,053 EV/µL was always significantly associated with higher in-hospital mortality (OR 5.1, 95% CI 1.4-20.0, p = 0.01). The present findings highlight the potential interest of detecting EVs expressing E-selectin (CD62) to discriminate Covid-19 patients at the time of hospital admission and identify individuals with higher risk of fatal outcome.

ABSTRACT

Recently, an article by Seneff et al. entitled "Innate immunosuppression by SARS-CoV-2 mRNA vaccinations: The role of G-quadruplexes, exosomes, and MicroRNAs" was published in Food and Chemical Toxicology (FCT). Here, we describe why this article, which contains unsubstantiated claims and misunderstandings such as "billions of lives are potentially at risk" with COVID-19 mRNA vaccines, is problematic and should be retracted. We report here our request to the editor of FCT to have our rebuttal published, unfortunately rejected after three rounds of reviewing. Fighting the spread of false information requires enormous effort while receiving little or no credit for this necessary work, which often even ends up being threatened. This need for more scientific integrity is at the heart of our advocacy, and we call for large support, especially from editors and publishers, to fight more effectively against deadly disinformation.

ABSTRACT

This concept paper addresses specific challenges identified in the UN 2030 Agenda Sustainable Development Goals (SDG) as well as the National Health Policy of India (NHP-India) and the Ministry of Health Policy of UAE (MHP-UAE). This policy calls for a digital health technology ecosystem. SDG Goal 1 and its related objectives are conceptualized which serves as the foundation for Virtual Consultations, Tele-pharmacy, Virtual Storage, and Virtual Community (VCom). SDG Goals 2 and 3 are conceptualized as Data Management & Analytical (DMA) Architecture. Individual researchers and health care professionals in India and the UAE can use DMA to uncover and harness PHC and POC data into practical insights. In addition, the DMA would provide a set of core tools for cross-network initiatives, allowing researchers and other users to compare their data with DMA data. In rural, urban, and remote populations of the UAE and India, the concept augments the PHC system with ICT-based interventions. The ICT-based interventions may improve patient health outcomes. The open and flexible design allows users to access various digital materials. Extendable data/metadata format, scalable architecture for petabyte-scale federated discovery. The modular DMA is designed using existing technology and resources. Public health functions include population health assessment, policy development, and monitoring policy implementation. PHC and POC periodically conduct syndromic surveillance to identify population risk patterns. In addition, the PHC and POC deploy medical and non-medical preventive measures to prevent disease outbreaks. To assess the impact of social and economic factors on health, epidemiologists must first understand diseases. Improved health due to compliance with holistic disease treatment plans and access to scientific health information.

ABSTRACT

Malaysian millennials are reported to be struggling with stress and well-being issues at the workplace during the last decade. Drawing from the Self-Determination Theory, this study examines whether flexible working arrangements (FWAs) could impact millennials' mental health, well-being and productivity at the workplace. A self-administered questionnaire was distributed to 400 millennials in the services sector during the COVID-19 period. Findings revealed that FWAs influenced millennials' mental health, well-being, and productivity as millennials value job autonomy and a friendly work ecosystem as an ideal workplace. This sees a compelling case for Malaysian businesses to adopt FWAs in a more structured way in line with Sustainable Development Goal-3.

Sustainable Development Goal-3 attempts to achieve health and well-being for all. Due to Covid-19 pandemic the general population across the globe have witnessed adverse mental health problems. This posed multiple challenges in ensuring wellbeing for all. In this context, this study is attempted to explore the relationship between big five personality traits and depression among general population during COVID-19 pandemic and the mediation role of health anxiety and coronavirus anxiety in relationship between personality and depression. Incidental sampling technique is utilized to collect the data using Google Form. Participants (N=216) filled up the questionnaires such as Big Five Inventory -10 Whitely Index 14, Coronavirus Anxiety Scale and Patient Health questionnaire-9. Results revealed that extraversion, agreeableness and conscientiousness factors of personality had negative relationship with health anxiety, coronavirus anxiety and depression where as neuroticism had significant positive association. Multiple regression analysis revealed that 9% of variance in depression can be explained by personality factors and 32.5% of variance was accounted by health anxiety and corona virus anxiety with respect to depression. Parallel mediation analysis indicated that extraversion factor had significant negative total effect on depression where as neuroticism factor had significant positive total effect on depression. The results and implications are discussed in the article. © The Electrochemical Society

Background: Several studies have suggested the possibility of obtaining specific respiratory benefits by experiencing forests and other natural resources. Despite this, forests have never been considered according to such potential. This study aims to compare municipalities by considering the absence/presence of tree species generating ‘above threshold’ potential respiratory benefits. Methods: The autonomous Region of Friuli Venezia Giulia in Italy has been assumed as a research area. The natural resource based view (NRBV), postulating the strategic role played by natural resources in achieving both above-average (thus ‘valuable’) and ‘concentrated’ (thus ‘rare’ among competitors) performance, has been adopted. The literature reviews dealing with potential respiratory benefits of biogenic organic compounds (BVOCs) emitted by trees, published within the ‘forest therapy’ research field, have been adopted. Three analysis models rating tree species by their potential respiratory benefits in ‘holistic-general’ (P1), ‘particular’ (P2), and ‘dynamic” terms (P3) have been outlined. The resulting overall potentials of tree species have been assessed by adopting the well-rooted Hollerith distance (HD) model. Tree species have been rated “1” when they satisfy one or more of 58 potential respiratory benefits. Municipalities have been ranked by considering the surface area covered by forest types whose dominant tree species achieve above-average potential respiratory benefits. QGIS software has been adopted to geographically reference the results obtained. Results: (P1) Valuable municipalities include those covered by both coniferous and deciduous forests;(P2–3) Municipalities achieving the highest potential respiratory benefits, in both particular and dynamic terms, have been mapped. Discussion: Forest-based initiatives that are running in the preselected municipalities can be both further improved and diversified in a targeted way. Conclusions: Despite some limitations mostly embedded in the concept of ‘model’, this study allows scholars to reduce uncertainties when locating municipalities in which to conduct local-scale experiments.

ABSTRACT

This article critiques the new Theory of Change (ToC) on mental health published by the UK’s Department for International Development (DfID) in the last fortnight of its existence. The ToC offers development actors a framework for better support of beneficiaries with mental health conditions and psychosocial disabilities – given disappointingly scant attention by the sector to date. Yet, 70 per cent of mental disorders occur in low- and middle-income countries (LMICs), with a 22 per cent prevalence in fragile and conflict-affected states. Globally, mental ill-health is estimated to affect almost one billion people. Its intersectionality with poverty and physical health has been brought into sharp focus by the current COVID-19 pandemic which has magnified the underlying social and environmental stressors of mental health. DfID’s ToC provides a conceptual framework for improving mental health globally, with an overarching vision of the full and equal exercise of all human rights by those affected by mental health conditions and psychosocial disability. The framework incorporates a rights-based approach with user-participation embedded in five critical change pathways to outcomes. The article analyses the ToC, provides an overview, highlights gaps and comments upon how DfID might have improved clarity for development actors seeking to realise its vision.

ABSTRACT

The 2030 Agenda for Sustainable Development adopted by the United Nations in 2015 recognizes noncommunicable diseases (NCDs) as a major public health challenge. Sustainable Development Goal (SDG) 3 includes target 3.4 to reduce premature NCD mortality by one-third by 2030. This review article analyzes the progress towards the attainment of targets within 3.4, the gaps in meeting the targets, and implementation challenges correlated with those gaps. A literature review was performed in September 2020 to identify the published literature and data discussing the SDGs and NCDs, its progress since 2015, and the associated challenges. The analysis reveals SDG target 3.4 is interrelated to at least nine SDGs. There have been many positive SDG initiatives, but the progress has been slow. Data from various countries show that only two out of the ten NCD progress indicators are being met by at least half of the 176 countries who signed the SDGs. The ongoing COVID-19 pandemic is expected to further aggravate the prevalence and hinder the progress towards the achievement of goals and the targets of the SDGs. The next decade is critical to advance progress on reducing NCDs across countries. The article concludes with a commentary and recommended actions. A combination of prevention, early detection, and treatment are the key to achieve the SDG 3.4 targets. Increased funding and commitments at international and national levels are required to bring about the transformative changes.

The threat of plastic waste pollution in African countries is increasing exponentially since the World Health Organisation declared the coronavirus infection as a pandemic. Fundamental to this growing threat are multiple factors, including the increased public consumption for single-use plastics, limited or non-existence of adequate plastic waste management infrastructures, and urbanisation. Plastics-based personal protective equipment including millions of surgical masks, medical gowns, face shields, safety glasses, protective aprons, sanitiser containers, plastics shoes, and gloves have been widely used for the reduction of exposure risk to Severe Acute Respiratory Syndrome (SARS) Coronavirus 2 (SARS-CoV-2). This paper estimates and elucidates the growing plethora of plastic wastes in African countries in the context of the current SARS-CoV-2 pandemic. A Fourier transform infrared (FTIR) spectral fingerprint indicates that face masks were characterised by natural and artificial fibres including polyester fibres, polypropylene, natural latex resin. Our estimate suggests that over 12 billion medical and fabric face masks are discarded monthly, giving the likelihood that an equivalent of about 105,000 tonnes of face masks per month could be disposed into the environment by Africans. In general, 15 out of 57 African countries are significant plastic waste contributors with Nigeria (15%), Ethiopia (8.6%), Egypt (7.6%), DR Congo (6.7%), Tanzania (4.5%), and South Africa (4.4%) topping the list. Therefore, this expert insight is an attempt to draw the attention of governments, healthcare agencies, and the public to the potential risks of SARS-CoV-2-generated plastics (COVID plastic wastes), and the environmental impacts that could exacerbate the existing plastic pollution epidemic after the COVID-19 pandemic.

ABSTRACT

This viewpoint paper argues for the need for more socially sustainable care systems that can better contribute to equitable utilization of health care in a post-pandemic era. Health care systems in developed nations need to rethink their role, particularly with regard to the achievement of Sustainable Development Goal 3 (SDG 3) as well as becoming more sustainable societies. Socially sustainable care systems will recognize that systemic factors and processes (social, economic, environmental, cultural) need to be addressed simultaneously in order to achieve health equity. Moreover, these systems are likely to be of paramount importance for post-COVID-19, because of the potential increase in demand for health care due to forgone health care and the increased burden of chronic diseases as spillover effects related to COVID-19 mitigation interventions.
